Unsubscribe from email alerts.

Accepts either:
- `token` query param (proves ownership without login — used by email links), OR
- `Authorization: Bearer <session>` header (proves ownership via login — used
  by the account page's unsubscribe button).

Requiring a token-only flow used to silently break the account-page button
and surface as a misleading "Unsubscribe token required" error, prompting
spam complaints from logged-in users who just wanted off the list.
