Fetched when the operator's bridge leg is answered; returns TwiML that
dials the server-stored dispatch target for ?dcid=. The target is read
from the DB row, never from the request, so the answered call can only be
connected to the number we resolved server-side. Unknown/absent row ⇒
hang up rather than dial anything.
