Insert one hand-written entry. Auto-milestone rows have a separate
code path in the scheduler — this endpoint is for the operator.

- ChangelogIn: Curated changelog entry payload. The auto-milestone path doesn't go through this — it INSERTs directly with milestone_source/value set.
