`GET /health`

## Request

Health check endpoint with database connectivity test.

Readiness route: ``status`` is ``degraded`` with HTTP 503 whenever the
database probe does not report ``connected``; ``details.database_error_code``
carries the typed code. Liveness is ``/health/live`` and ``/healthz``.
