9811d04c — remove a persisted custom plugin by name.

Name comes from ``?name=`` or a ``{"name": ...}`` JSON body. ``?hostname=X``
scopes to that machine. Built-in slot overrides are left untouched (only a
non-built-in entry with a matching name is dropped). Returns the updated
``custom`` list. Removing a name that isn't a custom plugin is a no-op 404.
