Force an error response to exercise the 5xx counter + admin alerting.

Gated by ``MERIDIAN_ENABLE_ERROR_TEST=1`` so it only exists on preview /
staging — never on prod. ``kind=exception`` (default) raises an unhandled
Exception so the middleware logs the traceback and synthesizes a 500.
``kind=500`` returns a raw HTTPException(500) so the middleware records
the response status without going through the exception path.
