Ingest a ServiceTitan form-fill booking as a Lead.

Called by service-titan-service's bookings ETL dispatcher. The org comes from
the request body, not the auth context; only compare when the caller carries
an organization. Dedup happens inside ``LeadIngestionHandler``.
