`GET /gtm-internal/post-call-packages`

## Request

Post-call docs that HAVE a generated `package` (JSONB key-presence
filter — dark-lane docs without one never appear), newest first, with the
account label/owner and the compact per-stage valid/invalid summary.
`account_id` (spine id) optionally narrows to one account — matching the
doc's spine anchor, or its domain for pre-dual-write docs.
