Kickoff (analysis | report | investigation) on an APPROVED task.add —
constraint 8's D3-by-command. v1 records the intent on the apply_log in
BOTH modes (live entries are additionally `kickoff_pending`); the actual
enqueue through the existing run adapters is a documented follow-up.
