Update an existing assignment's run-order priority for one scope.

The CLI calls this when ``settings set branch_sql --order`` changes the run
order of a hook that already exists (the policy PATCH only touches config).
Scope is named in the body via (scope_type, scope_id) so a policy assigned
to several scopes stays individually addressable.
