Record the self-reported "How did you hear about us?" (2026-10-01).

Write-once, like signup-source: a conditional UPDATE lands only while
``heard_about`` is NULL, and "skipped" counts as an answer so the question
is never asked twice. Refusals are 200s with ``recorded: false``.
