Operator recovery for wedged incidents.

Flips a stuck incident (codex_needs_attention, codex_failed,
gamma_validation_failed_codex_dispatched, gamma_fix_validating,
codex_dispatched, codex_retry_dispatched, manual_triage_required)
to `gamma_fixed_pending_prod_approval` and sends the approval email.

Use when the state machine got stuck but a verified gamma fix landed —
e.g. a workflow wait-step timed out before CI completed (the
2026-05-11 50665877dea0e3dd cycle). Requires a reason in the body for
the audit trail; pass `gamma_validation_run` / `gamma_validation_url`
when you've verified the Deploy run yourself.

Returns 409 if the incident is already at `gamma_fixed_pending_prod_approval`
(use approve-prod instead) or past it (prod_promotion_dispatched / stable).
