List the destructive schema changes YOU proposed and where each stands: requested (waiting on
the database owner), approved, applied, denied, or canceled. Use this after schema_apply reports
a change needs approval — read the resolution note on a denial and adjust rather than retrying
the same spec. Each entry carries the exact DDL and the blast radius that was measured.
