`GET /api/billing/candidates/{candidate_id}/total`

Running pipeline cost for one candidate. Drives the per-candidate
cost widget on the candidate / interview detail pages.

- CandidateRunningTotalResponse: Running cost for a single candidate's pipeline. Used by GET /api/billing/candidates/{candidate_id}/total to drive the per-candidate cost widget (PipelineCostBadge).
