Send a pending copy back to its author with a reason.

The copy stays PENDING and is flagged ``needs_changes`` — the author has
to tell "nobody has looked at this yet" from "looked at, and sent back".
A live copy, if there is one, is untouched and stays up.

- `token` string, nullable: Review link token, as minted into the Slack ping. Scoped to this assistant and to one reviewer; ignored when the caller is already signed in as a reviewer.
