Approve a pending copy and put it live.

Promotion is atomic: whichever copy was live is retired in the same
transaction, so the public never sees two versions or none.

- `token` string, nullable: Review link token, as minted into the Slack ping. Scoped to this assistant and to one reviewer; ignored when the caller is already signed in as a reviewer.
