`POST /api/mcp/generate_cover`

Generate a non-spoiler cover image for a chapter.

Reads the chapter, plans an establishing scene that sets the chapter up
without revealing its climax or twist, grounds it on the featured cast's
reference art (gpt-image-2 multi-reference edit — pose sheet preferred
per character), and renders it through DesignForYou.
Returns `{cover_job_id, status: "running"}` — poll
`get_chapter_cover_status` until terminal. The generated cover is saved
on the chapter (`output_artifacts.chapters[n].cover`).

## Request body

- GenerateChapterCoverInput
  - `job_id` string, required: The story job id.
  - `chapter` integer, required: 1-based chapter number to cover.

## Response `200`

Successful Response

- GenericToolResponse: Fallback for tools whose payload is still being tightened.
