Self-serve cancel-anytime. Idempotent + never flag-gated — revocation is
a safety operation that must never be stranded.

Effect order (the effect first, the audit row second — a dropped audit row
must never leave a publisher un-revoked): stamp ``takedown_at`` +
pause ACTIVE intake sources + commit, THEN append the ledger revocation row,
THEN notify ops (best-effort). ``distribution_authorized`` honors
``takedown_at`` as the always-wins revocation, so narration + managed
distribution stop immediately for this publisher.
