The publisher's on-file Audio Authorization acceptance + revocation state.

``revoked`` is the effective flag — ``takedown_at`` (the always-wins
revocation) OR a ledger revocation row. The dashboard Agreement card renders
``on_file`` (the receipt) and toggles the "Cancel authorization" affordance
on ``revoked``.
