Consume a double-opt-in confirm link.

Hashes the token, matches a ``pending`` row, flips it to ``confirmed``,
then 302-redirects to the signal page with ``?confirmed=1``. An
invalid/expired/consumed token redirects with ``?confirmed=0`` (no
enumeration, no error page).
