Stream a per-article cover image from R2 when the bucket is
private. Cached server-side via the standard R2 layer; the
response carries an immutable 1-year Cache-Control so the CDN/
browser never has to fetch the same key twice.

Used when ``r2_public_url_base`` isn't set — frontend's
``cover_image_url`` points here instead of a direct CDN URL.
