One E.164 → person map for phone surfaces.

Sources, in name priority order: canonical contacts (the identity graph),
team members, registered simulation counterparties, and the team's own
lines. The `simulated` flag rides independently of the name so a simulated
party never masquerades as a real person (Lane E), and `line` marks numbers
the team itself owns.
