Paginated, filterable MCP call log for the Activity Log page.

Ordered ts DESC with `uuid` as a stable tiebreak. Cursor pagination
via opaque `base64(ts_iso|uuid)`. Org owners/admins see every row;
plain members are scoped to their own `user_uuid` server-side — a
member querying another user's rows gets an empty page rather than
a 403 (parity with the chart endpoints).
