CLOSE a task with the structured completion QA survey (mcp:*) — the preferred
way to finish a task, instead of a free-form note. Fill it concisely: a verdict,
a one-line summary, the metrics (numbers → a table), the QA checks you verified,
a short journal, and any follow-ups. The verdict sets the status. Unknown id 404.
