Scan-run HISTORY, newest first — one item per ingest (mcp:*, read-only, 0 credit).

MCP twin of `GET /v1/projects/{id}/code-scan/runs`. Each item is one run's
recap: `counts{new, persisting, treated, gone}`, `submitted`, `via`, actor.
Cursor pagination — read `cursor_next`, pass it back as `cursor` until null.

An EMPTY history means no scan has ever run here — an unmeasured project, not
a clean one. `summary` shows only the LAST run; this shows all of them.
