Finding-exclusion rules (mcp:*, read, 0 credit). MCP twin of
`GET /v1/projects/{id}/code-scan/exclusions`, trimmed to `{type, value}` — all a
scanner needs.

A scan reads these to DROP ignored files/folders/strings from its local report
and its counts, matching the console, which masks them by default. Each rule is
one of: `string` (exact literal), `file` (exact repo-relative POSIX path), or
`folder` (every path under `value + '/'`).
