`DELETE /api/orgs/{slug}/members/{user_id}`

Remove a member — and every per-app grant they hold in this org.

The cascade lives in `orgs_engine.remove_member`. Without it, revoking
membership would leave `user_apps` rows intact and the person would keep
full access to the apps they were on: the removal would look like it
worked and would have done nothing.
