摩擦點診斷狀態機（MCP 用）。

Actions：
- `start`：開新 session，回 purpose_question。
- `submit_purpose`：用戶選 A/B/C 後回 4 個深挖題。
- `submit_answers`：收集 4 答案後回 hypothesis 模板（用戶預覽）。
- `confirm_hypothesis`：用戶確認（可帶 modified_hypothesis），status 轉 confirmed。
- `get`：查當前狀態（除錯用）。

Session_id 從 start 拿，後續呼叫帶著。跳步會 400。
