MCP Streamable HTTP transport with JSON responses. Protocol 2025-11-25, 2025-06-18 or 2025-03-26 through the initialize handshake; protocol 2026-07-28 without a handshake, selected by the MCP-Protocol-Version header (server/discover, per-request _meta, Mcp-Method and Mcp-Name headers). GET and HEAD answer 405 with Allow: POST; no server-sent event stream is offered.

- `202`: Notification or client response accepted; empty body
- `400`: Malformed body, unsupported MCP-Protocol-Version header, or a 2026-07-28 request refused with a JSON-RPC error (-32600, -32602, -32020 header mismatch, -32022 unsupported version)
- `403`: Invalid supplied Origin or insufficient credential permissions
- `404`: A 2026-07-28 request for a method this server does not implement (JSON-RPC -32601)
