  - `provider_id` string, uuid, required: The provider's Silna identifier
  - `provider_group_id` string, uuid, required: The provider group's Silna identifier
  - `name` string, required: Location name
  - `address_line_1` string, required: Primary address line
  - `address_line_2` string, nullable: Secondary address line
  - `city` string, required: City name
  - `state` string, required: Two-letter state code
  - `zip_code` string, required: ZIP code
  - `source_id` string, nullable: An API client's (internal) identifier for a service location

## Response `201`

- V1ServiceLocationCreateResponse
  - `id` string, uuid, required: The newly created service location's Silna identifier
