  - `id` string, uuid, required: The provider's Silna identifier
  - `legal_name` string, required: Legal name of the provider
  - `dba` string, required
  - `source_id` string, nullable: An API client's (internal) identifier for a provider
  - `silna_url` string, required: URL to view this provider in the Silna Health app
