    - `treatment_codes` V1TreatmentCodeCreate[], required: List of treatment codes
      - `code` string, required: Treatment code (e.g. a CPT code)
      - `treatment_code_modifiers` V1TreatmentCodeModifiersCreate[], required: List of treatment code modifiers
        - `code` string, required: Modifier code
    - `units` string, required: Approved units (decimal string format)
    - `unit_type` 'VISITS' | 'CPT_UNITS' | 'HOURS' | 'DAYS', required
    - `time_frame` 'DAILY' | 'WEEKLY' | 'MONTHLY' | 'AUTHORIZATION_PERIOD' | 'TOTAL' | 'SIX_MONTHS' | 'YEARLY'

  - `approval_letter_file_ids` string[], nullable: List of approval letter file IDs to trigger OCR. Mutually exclusive with manual approval details.
