Mint an additional emitter token for an already-approved producer.

Multiple emitter tokens per producer is the supported pattern for
multi-instance agents (e.g. several runners of the same skill). All
tokens for the producer attribute events to the same producer name;
they differ by `producer.run_id` / `producer.host` set on each emit.
