The latest faithfulness readings for an environment: the env-wide model
(scope=_model) + per-process. For any scope with a GOVERNED spec the reading is
RECOMPUTED live (so the trust-meter can't drift from a stale POST); scopes with
only a legacy reading are returned as stored. Backs the UI trust-meter.
