Liveness probe. 200 while the process is alive — INCLUDING during the
background startup warmup (the pod is healthy, just not yet ready for data;
see specs/nonblocking-startup/v0.1.md). Returns 503 only when the warmup
has FATALLY failed, so k8s restarts the pod instead of leaving it
permanently un-ready. Backward-compatible: `ok` is always present; `version`
/ `spec_version` are additive.
