Index: every skill with its served `latest` version + content_hash.

Mirrors `GET /polaris-baselines`. Without this there's no enumeration entry
point — a caller must already know a skill_id (the per-skill routes 404 on
unknown ids), and a bare `GET /agent-skills` would fall through to the SPA
catch-all and return dashboard HTML.
