Record the erasure of an account whose record was already removed OUTSIDE the erasure
path (spec §3.2): tombstone + cleanup of what still links the uid + `account.deleted` +
the ledger line. `dry_run` defaults to TRUE — a real run needs `dry_run: false`, `confirm`
equal to the uid, and a `reason`. `handle` / `trial_email` (optional, from a backup) only
feed the keyed hashes and are never stored or echoed.
