# Create a new Form Response

`POST /formresponse.json`

#### OAuth Scope
This endpoint requires the following OAuth scope **manage_forms**.

			
			
#### Record UUID
UUID is optional for record creation. If no UUID is supplied, a UUID will be automatically generated for the new record and returned in the `x-record-uuid` response header.

## Request body

- FormResponse
  - `uuid` string, uuid — Unique identifier for this record
  - `active` 0 | 1 — Record active/deleted flag. Valid values are [0,1]
  - `edit_date` string — Timestamp at which record was last modified
  - `form_uuid` string, uuid
  - `staff_uuid` string, uuid
  - `regarding_object` string
  - `regarding_object_uuid` string, uuid
  - `field_data` string — JSON array of form answers captured at submission time.
  - `timestamp` string
  - `form_by_staff_uuid` string, uuid
  - `document_attachment_uuid` string, uuid
  - `asset_uuid` string, uuid

## Response `200`

## Other responses

- `400` — Bad Request - The request is malformed or contains invalid parameters
- `401` — Unauthorized - Authentication credentials are missing or invalid
- `403` — Forbidden - You don't have permission to create this resource
- `429` — Too Many Requests - You have exceeded the rate limit
- `500` — Internal Server Error - An unexpected error occurred on the server
