`GET /api/documents/subtopics`

Return the browsable subtopic glossary for a subject/topic.
Always derives from chapter names in heading_hierarchy (level 1 then 0),
with a section_title fallback when hierarchy is sparse.
The Subtopic table is intentionally skipped — it often contains low-quality
data (sentence fragments from section_titles) rather than clean chapter names.
