`GET /api/concepts/{concept_id}/explain`

Get a concept explanation card.
When assessment_id is provided, generates a targeted explanation based on
the actual question the user got wrong — not a generic concept overview.
Without assessment_id, checks memory_semantic cache first; generates via LLM on cache miss.
