`POST /api/candidate-accounts/request-code`

Email a 6-digit code. Always 202, never an enumeration oracle.

The IP limit here is belt-and-braces on top of the DB-backed per-email
limit in the service (which is the one that survives multiple replicas).
429 is the ONE non-202 outcome, because silently dropping a code the
candidate is waiting for is worse than telling them to wait.
