GROSS EXPOSURE CAP — the most the desk's open positions may be worth,
as a FRACTION of the desk's equity (1.5 = 150%), counted across every
bot. Unlike `max_open_usd` it MOVES WITH THE ACCOUNT, which is the point.
Null clears it. Refusals are `skipped_max_gross_exposure`.
