Observations and check-ins across this clinician's caseload since their last visit per child.

A check-in is in the set so a parent's reply on it (a comment on the
check-in's own entry) resurfaces it; the clinician's own, unanswered, is
held out by the function's own-post rule (`20261009021338`). A document
comes in only with `documents`, and only through a comment (#14447).

The join, cutoff comparison, per-child cap and unread count all happen in a
single SECURITY INVOKER SQL pass (`get_clinic_new_updates`) - RLS still gates
`child_profiles`, `timeline_entries`, and `user_timeline_read_state`.

## Query parameters

- `limit` integer: Max entries per child
- `include_digest` boolean: Attach today's Haiku catch-up digest
- `documents` boolean: Include documents brought back by comments (#14447). Sent by the catch-up surfaces only.
