The client list is built from individual **and** org-level grants, and only
an individual grant (or being the child's family) permits authoring. Without
this the org-only case is discovered at the end: the clinician dictates,
uploads the whole file, and is told to try again about something that will
never succeed. Asked when the screen opens, answered by the gate itself.
