Approve a proposed entry so it starts being injected.

This click is now the ONLY way anything reaches the prompt — recurrence
stopped promoting on 2026-09-17 — so it is also the last place a false
fact can be stopped. 409 with the disagreement named when the entry
contradicts the account roster; the fix is then the memory or the mirror,
and the caller can see which.
