`GET /api/projects/{project_id}/papers/{paper_id}/{generation_index}`

Download one paper version (302 → presigned S3 URL, attachment).

``fmt`` is the preferred format ('pdf' default, or 'md'). When the
preferred format isn't in S3 (e.g. PDF generation failed) we transparently
fall back to the other so the user still gets a file — from their
perspective the download behaves the same, a .md just pops out instead.
