`GET /api/auth/oauth/{provider_name}/start`

Kick off the login: stash state in a cookie, bounce to the provider.

`rerequest` is set only by our own callback, when the provider came back
missing a permission we require. It re-opens the dialog asking for the
declined permission again, and marks the state so we can tell a retry from
a first attempt and never bounce more than once.
