# Patient Provider Map

`GET /api/v2/patients/{particle_patient_id}/provider_map`

## Request

- Base URL: `https://sandbox.particlehealth.com`
- URL: `https://sandbox.particlehealth.com/api/v2/patients/{particle_patient_id}/provider_map`
- Auth: API key in header `Authorization`

Returns the set of healthcare organizations associated with given patient across Particle's network. Each entry represents an endpoint organization enriched with directory information (NPI, address, managing organization). Returns an empty `providers` array if no records are found.This endpoint requires the Patient Provider Map feature to be enabled for your project. Reach out to your Particle Health representative to get set up.

    - `oid` string, required: Organization Identifier (OID) for the endpoint
    - `organization_name` string, required: Name of the endpoint organization
    - `network` string, required: Network source. Either CAREQUALITY or EHEALTHEXCHANGE
    - `last_seen` string, required: Most recent date a record was observed for this patient at this organization (YYYY-MM-DD)
    - `active` boolean: Whether the endpoint is currently active in the directory
    - `type` string: Organization type (e.g., Practice, Implementer)
    - `npi` string: National Provider Identifier for the organization
    - `address` string: Street address
    - `city` string: City
    - `state` string: State abbreviation
    - `postal_code` string: ZIP code
    - `lat` string: Latitude coordinate
    - `lng` string: Longitude coordinate
    - `managing_organization_name` string: Name of the managing (implementing) organization behind the endpoint
    - `managing_organization_oid` string: OID of the managing organization

- `401`: Unauthorized
- `404`: Not Found
- `500`: Internal Server Error
