Execute an MCP tool by name (rate-limited per CALLER by tier).

Origin-protected (see _mcp_origin_check) + tier-based rate limit.
Programmatic API-key callers bypass the origin check.

Successful Response

- AdminMcpToolResponse: `POST /api/mcp/tool` — execute MCP tool by name. Run 19 F4 backfill 2026-05-12. Wraps the MCP tool's native result in a status envelope. Rate-limited per IP by tier.
  - `status` string, nullable: 'ok' on success, 'error' on failure.
  - `tool` string, nullable: Echoed tool name on success.
  - `result` object, nullable: Native MCP tool result (tool-shaped).
  - `error` string, nullable: Error message on failure (unknown tool / invalid body / execution error).
  - `retry_after` integer, nullable: Seconds until retry allowed (only set on 429).
  - `limit` integer, nullable: Rate-limit ceiling for this caller's tier (only set on 429).
