QoQ ownership change analysis for a ticker across notable funds.

Classifies each fund's position as: new, increased, decreased, unchanged,
or exited. Useful for tracking institutional-holdings conviction shifts.

Data source: fund_holdings_snapshot table (13F-HR filings).

## Query parameters

- `ticker` string, required
- `quarters` integer: Number of quarters to analyze

Successful Response

- OwnershipChangesResponse: `GET /api/institutional-holders/changes` — QoQ ownership change buckets. `changes`, `increased`, `decreased`, `new_positions` are all iterated in frontend (CR-C2 2026-05-01). Error envelope at routes_analysis_fastapi/options_insiders.py:362 returns each as []; schema declares them required to match.

  - `decreased` object[], required: Funds that decreased their position QoQ.
